Since launching the Care Coordination and Health Coaching programs in 2018, Care Coordinators and Health Coaches have helped hundreds of employees/family members manage chronic health conditions. Comments from participants have been very positive:
“This has been an incredibly valuable benefit. With Lisa’s guidance, I have lost 15 pounds and counting. I feel one thousand times better. Lisa taught me how making tailored food choices, eliminated my morning fog.
I am SO grateful to Lisa!”
“Your support means more than I can express…thank you!”
“Having first the care coordinator and now a health coach has changed my life. The suggestions were practical. My care coordinator followed up by sending lots of great resources.”
“I have a very serious health condition and see many doctors and take a lot of medications as a result. The copays have really hurt me. I needed a special authorization for a medication and my care coordinator moved mountains to make this happen. I also needed an upcoming test that needed to be changed and she worked with everyone at the different offices to make that happen. She knows who to call and where to go next. When I am down she always finds a light at the end of the tunnel for me. I appreciate everything she does for me and she is so special to me. Thank you for this program and what it has done to help me.”
“My health coach did an excellent job regarding my health issues and personal needs. She is very understanding and went above and beyond the call of duty. Thank you so much for attending to me.”
