Data Coordinator

JOB SUMMARY:
Works as part of the DVACO Clinical team and supports work for Quality Improvement, Practice Transformation, Care Continuum, and Care Coordination teams to collect, process, integrate, analyze, distribute, and present data. This person will develop and maintain tools, descriptions, answers, and guidance for projects to improve clinical quality, utilization, and financial success for the ACO and its participating practices. Manages and meets reporting deadlines for assigned deliverables and ad hoc reports/needs. Role will provide administrative and project management support to the teams.

ESSENTIAL ACCOUNTABILITIES:

  1. Arranges subcommittee meetings (schedules, develops agendas, and compiles meeting notes) to support the clinical initiatives of these teams.
  2. Works with departmental clinical leadership to develop schedules, agendas, completes minutes/notes. Creates Desktops and standard work documents for departments
  3. Serves as coordinator of technical support regarding DVACO software including the DVACO Dashboard/NorthStar reporting requirements, Bamboo Health, and primary access manager (access, passwords, and troubleshooting) for DVACO ShareFile and external payor platforms.
  4. Together with the Clinical Quality Coordinator, responsible to set up and coordinate initial Payer Software (e.g., Compass, Evoke 360 etc.) for DVACO, employed providers and Provider Staff offices
  5. Leads activities for new and continuing existing EMR access for users and update all QI and other EMR access to external EMRs
  6. Functions to perform phone outreach and coordinates phone/ IVR/Texting outreach initiatives for DVACO members
  7. Collects data from disparate partners and from various sources to produce daily, weekly, and monthly reports to evaluate productivity and processes.
  8. Coordinates and submits all data related to accurate risk coding activities
  9. Coordinates non-clinical phone calls to support internal staff in responding to providers, and/or practice staff. May conduct research on resources.
  10. Performs Project management assignments as needed
  11. Participates in administrative support, data collection, analytic and submission requirements for MSSP QI submission requirements including Supplemental Data Process
  12. Run pivot tables and reports; help administer the clinical updates to our website, and help with the administrative functions of creating policies, curriculum/training documents, etc.
  13. Create and manage QI desktop process, support annual Preferred SNF application process, coordinate annual Web Interface activities, support payor requirements. Keeps the clinical team organized and timely.
  14. Retrieve and distribute data to produce department reports and help evaluate/track performance metrics.
  15. Support transition from Web Interface to eCQM submission for MSSP contract
  16. Coordinate with operations and analytic departments for technical and analytic requirements to meet all clinical initiatives and requirements
  17. Other duties as assigned.

Additional Knowledge, Skills, Abilities

  1. Experience with basic statistical analysis
  2. Ability to organize data in a meaningful way using detailed, summary, or graphical formats.
  3. Excellent verbal and written skills to interface with physicians, office staff, care management team, nursing staff, ancillary staff, vendors, outside customers, insurance companies and government agencies.
  4. Familiarity with EMRs and the operations of physician practice.
  5. Knowledge of medical terminology, clinical diagnosis, Medicare, and procedures generally acquired through education and/or experience in healthcare related environment preferred.

  • Since interaction with clinical staff may be required, exposure to other substances in these areas is possible.
