Liz Todd, PT, DPT
Manager of Care Continuum

As Manager of Care Continuum, Liz Todd is responsible for the development and implementation of programs supporting population health management and quality improvement across the care continuum. She collaborates closely with other clinical departments and health-system leadership to manage relationships with community-based providers. Initially joining the DVACO in May 2020 as the Post-Acute Care Coordinator, she was the DVACO’s post-acute clinical resource for physician practices, health system leadership and post-acute providers. Liz has a passion for end-of-life care and the use of clinical data analytics to drive improved patient outcomes.

Over her 15+ years of healthcare experience, she has led clinical quality improvement initiatives, clinical education, and interdisciplinary teams. Liz has worked in a variety of settings including acute care, inpatient rehabilitation, outpatient therapy and home health. Originally from Illinois, she received her Doctorate in Physical Therapy from Midwestern University and her Bachelor of Science from Bradley University. Liz lives in Chester County and enjoys reading, traveling, and spending time outdoors with her family.
