DVACO’s Practice Transformation Team works with the staff and providers at primary care and specialty care practices. The Practice Transformation Coaches support these practices transitioning from a fee-for-service reimbursement model to a fee-for-value model.

Our team has experience working across primary care, specialty care and large health systems. They have experience working with several different Electronic Medical Records (EMRs), developing workflows, educating and training practices on healthcare programs, and supporting performance improvement plans.

The goal of the Practice Transformation Team is to provide support to practices by assisting them in developing and improving their workflows and processes to address opportunities to facilitate a more proactive approach to population health management.

Meet the Team

Lauren Talley, Practice Transformation Coach
Lauren Talley, Manager of Practice Transformation & Network Development
Lauren Talley joined the DVACO Practice Transformation Team in 2018. Prior to joining the ACO, Lauren’s background included working as a practice manager in various specialties including OB-GYN, Bariatrics, General and Colorectal Surgery. Her experience working in an ACO practice, using various EMR systems, clinical workflows and practice operations has proven beneficial in her work with member practices. This work includes optimizing patient outcomes by analyzing data and improving processes and operations within the practices. In addition, Lauren has led different initiatives and created internal and external education documents on various healthcare programs, including Telehealth toolkits, to assist practices with implementation of telehealth during the Public Health Emergency. She often collaborates closely with other teams and departments within the DVACO to continue to accelerate the successful transformation to high quality value-based care for our patient populations.
Dawn Mancinelli, Practice Transformation Coach
Dawn Mancinelli, BSN, RN, Practice Transformation Coach
Dawn has been a registered nurse for over 30 years. Her clinical background includes medical-surgical, insurance home care coordinator, IV team/infusion therapy (including home infusion therapy and management), hematology/oncology infusion nurse, and primary care practice (transitional care management, care coordination, and PCMH project lead). She joined the DVACO in 2016 as a Nurse Care Coordinator, providing support to patients and physicians to improve health outcomes. In 2017, she transitioned to the Practice Transformation team and this has allowed her to expand her work to include providing support to our member practices and their staff. She has been able to use her skills to work with practices to obtain their PCMH recognition and meet ever changing MIPS requirements. Working with practices on quality improvement, closing patient gaps in care, improving care, and utilizing data to optimize key performance indicators in the value-based environment has taken her work full circle with the ultimate goal of improving the quality of patient care and the patient’s care experience.
Charles Pascale III, Practice Transformation Coach
Charles Pascale III, Practice Transformation Coach
Charles Pascale III, who joined the DVACO in March of 2020 is Licensed Practice Nurse and has been in the medical field for 18 years, starting out at Riddle Memorial Hospital. Riddle is where his passion for healthcare and patient care was recognized. Charles has spent 12 years in primary care settings and of those, 7 years managing and collaborating practice transformation. Charles enjoys working directly with practice leads and providers helping them with workflows as it relates to quality patient care.
Charles is married with 3 children. In his spare time Charles enjoys coaching his children in sports, traveling, Philadelphia sports and spending time at the beach with his family and friends. Charles has also been a volunteer firefighter since 1996. Charles and his family currently reside in Delaware County.
