One of the minimal requirements that must be met prior to consideration for becoming a post-acute partner with the DVACO is providing us with real time admission, discharge , and transfer information from your facility/organization for our shared populations. Preferred Outpatient Therapy Network

The Delaware Valley ACO is very pleased to announce our Preferred Outpatient Therapy Network. Our Preferred Organizations were selected to help our target population, our physicians, our hospitals and other healthcare collaborators identify Outpatient Therapy providers that achieve high standards for quality and optimal total cost of care for our target population. Our preferred network serves patients who need “in home” physical, occupational and/or speech rehabilitation therapy. These patients are either unable or it is too difficult for them to get out of their home to go to an outpatient therapy gym. If patients have skilled nursing needs in addition to “in-home” therapy needs, then please contact one of our preferred home care providers.

Preferred Hospice Organization Network

The Delaware Valley ACO is very pleased to announce our Preferred Hospice Network. Our Preferred Organizations were selected to help our patients and families, our physicians, our hospitals and other healthcare collaborators identify Hospice Organizations that achieve high standards for quality, caregiver satisfaction and demonstrate strong partnership and collaboration with partner organizations. Our preferred Hospice Organizations were identified based on criteria developed by our team of post-acute care experts and will be reviewed and updated regularly to assure continued high quality of our partners in the DVACO Preferred Hospice Network.

Preferred Home Health Organization Network

The Delaware Valley ACO is very pleased to announce our Preferred Home Health Network. Our Preferred Organizations were selected to help our patients, our physicians, our hospitals and other healthcare collaborators identify Home Health Organizations that achieve high standards for quality and demonstrate strong partnership and collaboration with partner organizations. Our preferred Home Health Organizations were selected by our team of post-acute care experts and are based on cost efficiency and quality metrics measures with our organizations demonstrating superior cost efficiency and quality metrics compared to our entire panel of home health providers.

Our preferred Home Health providers list will be reviewed and updated regularly to assure continued high quality of our partners in the DVACO Preferred Home Health Network.

Preferred Skilled Nursing Facility Network

The Delaware Valley Accountable Care Organization is proud to announce our Preferred Skilled Nursing Facility Network. Our Preferred Network list of skilled nursing facilities (SNFs) is developed to help our patients, our physicians, our hospitals, and other healthcare collaborators identify SNFs that achieve high standards for quality and demonstrate strong partnership and collaboration with partner organizations. Our preferred SNFs applied to participate in our Network and were selected by our team of post-acute care experts based on specific measures of quality, clinical services, use of technology, and partnership and collaboration.
